- For a personalized bridal cover, select Custom Cover and enter your first name, married last name, and maiden name.
- If you’re sending a Née Name Change Kit as a gift, select Gift Tag and enter the To/From information.
- Once your kit arrives, simply follow the step-by-step instructions to begin your name change.
